Pain is generally an unpleasant feeling in response to an event that either damages or can potentially damage the body’s tissues. There are four main steps in the process of feeling pain: transduction, transmission, perception, and modulation. The nerve cells that detect pain have cell bodies located in the dorsal root ganglia and fibers that transmit these signals to the spinal cord. The process of pain sensation starts when the pain-causing event triggers the endings of appropriate sensory nerve cells. This type of cell converts the event into an electrical signal by transduction. Several different types of nerve fibers carry out the transmission of the electrical signal from the transducing cell to the posterior horn of spinal cord, from there to the brain stem, and then from the brain stem to the various parts of the brain such as the thalamus and the limbic system. In the brain, the pain signals are processed and given context in the process of pain perception. Through modulation, the brain can modify the sending of further nerve impulses by decreasing or increasing the release of neurotransmitters.

Visit your doctor if you have low back pain. They will review your symptoms and health history and do a physical exam. Based on this information, they may perform tests. These include an X-ray, computerized tomography (CT) scan, or magnetic resonance imaging (MRI). Tests help to diagnose the cause or rule out other causes.

Both disc problems can cause nerve compression, inflammation, and pain.

Low Down on Low Back Pain Skirts & Skorts CT myelography employs traditional myelography (intrathecal instillation of a radiocontrast agent, followed by routine spinal radiographs) followed by CT of the spine. It may be of value when MRI studies are inconclusive, especially in the assessment of the relation between spinal nerve roots and the bony neural foramina. Although this technique involves an invasive procedure, it is the study of choice when there are absolute contraindications to the use of MRI (presence of an implanted electronic device such as cardiac pacemaker or metal object in the inner ear, eye, or brain, such as an aneurysm clip).
To help your doctor diagnose the source of low back pain, be specific in describing the type of pain, when it started, related symptoms, and any history of chronic conditions. Your doctor will probably not need to order X-rays, CT or MRI scans before starting treatment.

In particular, early use of imaging (either MRI or CT) is recommended for suspected cancer, infection, or cauda equina syndrome. MRI is slightly better than CT for identifying disc disease; the two technologies are equally useful for diagnosing spinal stenosis. Only a few physical diagnostic tests are helpful. The straight leg raise test is almost always positive in those with disc herniation. Lumbar provocative discography may be useful to identify a specific disc causing pain in those with chronic high levels of low back pain. Similarly, therapeutic procedures such as nerve blocks can be used to determine a specific source of pain. Some evidence supports the use of facet joint injections, transforminal epidural injections and sacroilliac injections as diagnostic tests. Most other physical tests, such as evaluating for scoliosis, muscle weakness or wasting, and impaired reflexes, are of little use.

your pain doesn’t improve or gets worse after 2 to 3 weeks. View All Treatment Supplies However, once the pain has eased or gone it is common to have further bouts (recurrences) of pain from time to time in the future. Also, it is common to have minor pains on and off for quite some time after an initial bad bout of pain. In a small number of cases the pain persists for several months or longer. This is called chronic back pain.

Several NIH-funded clinical trials and other studies in patients aim to improve treatment options and prevention strategies for chronic low back pain, as well as add to the evidence base about existing treatments. A multi-year multicenter study called the Spine Patient Outcomes Research Trial (SPORT) compared the most commonly used surgical and nonsurgical treatments for patients with the three most common diagnoses for which spine surgery is performed: intervertebral disc herniation, spinal stenosis, and degenerative spondylisthesis. SPORT represented the largest clinical investigation to date looking at treatment results for these disabling and costly causes of chronic low back pain.
Biofeedback is used to treat many acute pain problems, most notably back pain and headache. The therapy involves the attachment of electrodes to the skin and the use of an electromyography machine that allows people to become aware of and selfregulate their breathing, muscle tension, heart rate, and skin temperature. People regulate their response to pain by using relaxation techniques. Biofeedback is often used in combination with other treatment methods, generally without side effects. Evidence is lacking that biofeedback provides a clear benefit for low back pain.
