Keep your back as straight as you can, keeping your feet apart with one leg slightly forward so you can maintain balance. Bend only at the knees, hold the weight close to your body, and straighten the legs while changing the position of your back as little as possible.
If you are already suffering from chronic back pain or pain of any kind, you should understand that there are many safe and effective alternatives to prescription and over-the-counter painkillers, though they may require some patience. Here are some strategies I highly recommend:
Finally, these factors all result in an overall enforcement of good posture. With the help of a lumbar support, your back will slowly strengthen and learn to keep good posture without you having to do anything other than sit down.
Cigarettes aren’t just hell on your heart and lungs. Smokers have a higher incidence of recurring back problems, Dr. Katz says. The cause and effects of this are many. Nicotine restricts blood flow to vertebrae and disks, so they may age and break down more quickly. It may also interfere with the body’s ability to absorb and use calcium, leading to osteoporosis-related bone and back problems. You know what you have to do: Quit. Go to smokefree.gov to customize your own smoking cessation plan.
The Xtreme Comforts Back and Lumbar Cushion is designed to prevent unhealthy sitting positions by providing the proper support to maintain the natural curve of the spine. The design helps increase blood flow, prevents muscle fatigue and back tightness, and allows you to sit comfortably without pain. At 13 inches wide, the cushion design includes unique four-point quick snap straps that allow the cushion to be secured to any chair. The cushion is made from 100 percent Deluxe Body Active Memory Foam that conforms to your individual shape while maintaining uniform density. The cushion cover is crafted from 100 percent bamboo, providing superior air circulation and breathability to keep your back cool. The cover is removable and can be washed in the machine with cold water.
Ibuprofen (Advil, Nuprin, or Motrin), available over the counter, is an excellent medication for the short-term treatment of low back pain. Because of the risk of ulcers and gastrointestinal bleeding, with your doctor about using this medication for a long time.
An intervertebral disc has a gelatinous core surrounded by a fibrous ring. When in its normal, uninjured state, most of the disc is not served by either the circulatory or nervous systems – blood and nerves only run to the outside of the disc. Specialized cells that can survive without direct blood supply are in the inside of the disc. Over time, the discs lose flexibility and the ability to absorb physical forces. This decreased ability to handle physical forces increases stresses on other parts of the spine, causing the ligaments of the spine to thicken and bony growths to develop on the vertebrae. As a result, there is less space through which the spinal cord and nerve roots may pass. When a disc degenerates as a result of injury or disease, the makeup of a disc changes: blood vessels and nerves may grow into its interior and/or herniated disc material can push directly on a nerve root. Any of these changes may result in back pain.

Low back pain that lasts at least one day and limits activity is a common complaint. Globally, about 40% of people have LBP at some point in their lives, with estimates as high as 80% of people in the developed world. Approximately 9 to 12% of people (632 million) have LBP at any given point in time, and nearly one quarter (23.2%) report having it at some point over any one-month period. Difficulty most often begins between 20 and 40 years of age. Low back pain is more common among people aged 40–80 years, with the overall number of individuals affected expected to increase as the population ages.
For lumbar angle, there were no significant interaction effects. The main effects of Group and Epoch were not significant. There was a significant main effect of Condition (p = 0.006), such that there were differences between each of the conditions. The mean lumbar angle was 7.73° greater with the lumbar support compared to standing (95% CI; 5.15-10.31). The mean lumbar angle was 10.61° greater with the standard chair compared to standing (95% CI; 8.28-12.94). The difference between the lumbar support and standard chair conditions was 2.88° (95% CI; 1.01-4.75). The lumbar support condition was closer to neutral standing than was the standard chair in the lumbar spine. When testing for a significant effect in static stance, the lumbar angle between healthy individuals and patients with LBP was not significantly different. See Figure 5 for a graphical representation of the angle means in the standing, lumbar support and regular chair conditions.
The cushion is created using premium grade memory foam that works remarkably well in conforming to the contours of your spine. This not only keeps your lower back in check but alleviates pressure along your entire spine by forcing you to sit up straight. The material responds to pressure and body heat to become more supple as you sit. Moreover, it also has the wonderful property of quickly returning to its original shape once the pressure and heat is removed, making it easily usable for multiple people with different body dimensions.

The management goals when treating back pain are to achieve maximal reduction in pain intensity as rapidly as possible, to restore the individual’s ability to function in everyday activities, to help the patient cope with residual pain, to assess for side-effects of therapy, and to facilitate the patient’s passage through the legal and socioeconomic impediments to recovery. For many, the goal is to keep the pain to a manageable level to progress with rehabilitation, which then can lead to long-term pain relief. Also, for some people the goal is to use non-surgical therapies to manage the pain and avoid major surgery, while for others surgery may be the quickest way to feel better.
Experts believe TENS encourages the body to produce endorphins, and may possibly block pain signals returning to the brain. Studies on TENS have provided mixed results; some revealed no benefits, while others indicated that it could be helpful for some patients.
Not all portable lumbar supports are suited to the car. Some are simply too bulky or have straps that are too short. Even cushions that are made for cars may not fit all makes and models, so do your homework before buying.

Biofeedback is used to treat many acute pain problems, most notably back pain and headache. The therapy involves the attachment of electrodes to the skin and the use of an electromyography machine that allows people to become aware of and selfregulate their breathing, muscle tension, heart rate, and skin temperature. People regulate their response to pain by using relaxation techniques. Biofeedback is often used in combination with other treatment methods, generally without side effects. Evidence is lacking that biofeedback provides a clear benefit for low back pain.
One more noteworthy endorsement, with regards to this whole website and all of my books, submitted by a London physician specializing in chronic pain, medical education, and patient-advocacy (that’s a link to his excellent blog):
Myelogram is an X-ray study in which a radio-opaque dye is injected directly into the spinal canal. Its use has decreased dramatically since MRI scanning. A myelogram now is usually done in conjunction with a CT scan and, even then, only in special situations when surgery is being planned.

People routinely have no pain despite the presence of obvious arthritic degeneration, herniated discs, and other seemingly serious structural problems like stenosis and spondylolistheses. This surprising contradiction has been made clear by a wide variety of research over the years, but the most notable in recent history is Brinjikji 2015. There are painful spinal problems, of course — which was also shown by Brinjikji et al in a companion paper — but they are mostly more rare and unpredictable than most people suspect, and there are many fascinating examples of people who “should” be in pain but are not, and vice versa. Spinal problems are only one of many ingredients in back pain. BACK TO TEXT

2013 — Minor update: Added some great backup from a terrific surgeon blogger — who advocates for the necessity of testing placebo surgeries, exactly as I have in this section for many years. [Section: The back surgery placebo problem, and how it limits our knowledge of the effectiveness of back surgeries.]
