Driving – it is important to have proper support for your back. Make sure the wing mirrors are properly positioned so you do not need to twist. The pedals should be squarely in front of your feet. If you are on a long journey, have plenty of breaks – get out of the car and walk around.
The National Institute of Neurological Disorders and Stroke (NINDS) is a component of the National Institutes of Health (NIH) and is the leading federal funder of research on disorders of the brain and nervous system. As a primary supporter of research on pain and pain mechanisms, NINDS is a member of the NIH Pain Consortium, which was established to promote collaboration among the many NIH Institutes and Centers with research programs and activities addressing pain. On an even broader scale, NIH participates in the Interagency Pain Research Coordinating Committee, a federal advisory committee that coordinates research across other U.S. Department of Health and Human Services agencies well as the Departments of Defense and Veterans Affairs.
Back pain is a big topic because between the upper back and tailbone, there are 17 vertebral bodies, many joints, the sacrum and tailbone. Plus fibrous and muscular supporting structures, intervertebral discs, spinal cord and nerve roots, and blood vessels. A simple injury, such as a back sprain/strain from lifting and twisting simultaneously, can cause immediate and severe pain that is typically self-limiting.
Injections may also be used to numb areas thought to be causing the pain. Botox (botulism toxin), according to some early studies, are thought to reduce pain by paralyzing sprained muscles in spasm. These injections are effective for about 3 to 4 months.
ankylosing spondylitis inflammatory arthritis of spine and pelvis, mostly Long term back pain starting well before middle age and progressing slowly and erratically, improves with activity but not rest, prolongued morning stiffness, possible involvement of other areas. More common in men.

2015 — Science update: Added discussion of a popular acupuncture-for-back-pain paper, Cherkin 2009. Which is not fit to line a birdcage, in my opinion. [Section: The fascinating case of acupuncture, formerly a contender in low back pain therapy, but which has now miserably failed well-designed scientific tests.]

If you cannot manage the pain using the medicine you are currently prescribed, this may be an indication for a reevaluation or to go to an emergency department if your doctor is not available. Generally, this problem is best addressed with the doctor writing the prescription who is overseeing your care.
This myth of “mechanical” failure of the low back has many unfortunate consequences, such as unnecessary fusion surgeries — a common and routinely ineffective procedure — and low back pain that lasts for years instead of months or weeks. The seriousness of chronic low back pain is often emphasized in terms of the hair-raising economic costs of work absenteeism, but it may well be far worse than that — a recent Swedish study shows that it probably even shortens people lives.8 The stakes are high. “Tragedy” is not hyperbole.
Most people who develop low back pain that comes on suddenly (acutely) have nonspecific low back pain. If there are no other associated symptoms and the pain is not too bad, many people are confident to just ‘get on with it’ and treat it themselves – and indeed most get better quickly. However, if in doubt, see your doctor for a check-over and advice.
