When a herniated disc is compressing the nerve roots, hemi- or partial- laminectomy or discectomy may be performed, in which the material compressing on the nerve is removed. A mutli-level laminectomy can be done to widen the spinal canal in the case of spinal stenosis. A foraminotomy or foraminectomy may also be necessary, if the vertebrae are causing significant nerve root compression. A discectomy is performed when the intervertebral disc has herniated or torn. It involves removing the protruding disc, either a portion of it or all of it, that is placing pressure on the nerve root. Total disc replacement can also be performed, in which the source of the pain (the damaged disc) is removed and replaced, while maintaining spinal mobility. When an entire disc is removed (as in discectomy), or when the vertebrae are unstable, spinal fusion surgery may be performed. Spinal fusion is a procedure in which bone grafts and metal hardware is used to fix together two or more vertebrae, thus preventing the bones of the spinal column from compressing on the spinal cord or nerve roots.

While additional authors have investigated the effect of various support systems on bodily symptoms, much of this work has been performed on healthy individuals. Aota et al.  measured the biomechanical effects and comfort levels when using a lumbar support cushion that inflated from 0.5 to 8.0 cm thick in a continuous passive motion chair. They noted significant improvements in the subjective measures of LBP, stiffness and fatigue with use of the system in both static and dynamic states. Conversely, Carcone and Keir  noted that, while a lumbar pad measuring 9 cm thick best maintained the lumbar lordosis in sitting, participants tended to complain that it pushed their body forward, the result being a centre of pressure (CoP) that was more anteriorly located on the seat pan. In their study, participants also reported that configurations with less lordosis (i.e., less than 3 cm) were more comfortable . Portable devices that do not account for the bulk of posterior pelvic soft tissue volume may push the lower body forward and distort the intended relationship between the seat pan features and the body . The preferred degree of lordosis may be related to the pain state of the individual , in that comfort may be affected by the angular change as well as the interaction between the buttocks and the seat pan.

Acupuncture, yoga and massage can improve chronic back pain, although the evidence for those interventions is weaker, according to the American College of Physicians’ U.S. guidelines for the diagnosis and treatment of chronic low-back pain.
The treatment of lumbar strain consists of resting the back (to avoid reinjury), medications to relieve pain and muscle spasm, local heat applications, massage, and eventual (after the acute episode resolves) reconditioning exercises to strengthen the low back and abdominal muscles. Initial treatment at home might include heat application, acetaminophen (Tylenol) or ibuprofen (Advil, Motrin), and avoiding reinjury and heavy lifting. Prescription medications that are sometimes used for acute low back pain include anti-inflammatory medications, such as sulindac (Clinoril), naproxen (Naprosyn), and ketorolac (Toradol) by injection or by mouth, muscle relaxants, such as carisoprodol (Soma), cyclobenzaprine (Flexeril), methocarbamol (Robaxin), and metaxalone (Skelaxin), as well as analgesics, such as tramadol (Ultram).
Increasing general physical activity has been recommended, but no clear relationship to pain or disability has been found when used for the treatment of an acute episode of pain. For acute pain, low- to moderate-quality evidence supports walking. Treatment according to McKenzie method is somewhat effective for recurrent acute low back pain, but its benefit in the short term does not appear significant. Lumbar support belts: Workers who frequently perform heavy lifting are often required to wear these belts. There is no proof that these belts prevent back injury. One study even indicated that these belts increased the likelihood of injury.
