The idea that the spine is fragile is an unjustified but deeply held belief, based on ignorance of a complex subject and on an obsolete mechanical view of biology that has dominated medical thinking for centuries.

After your initial visit for back pain, it is recommended that you follow your doctor’s instructions as carefully as possible. This includes taking the medications and performing activities as directed. Back pain will, in all likelihood, improve within several days. Do not be discouraged if you don’t achieve immediate improvement. Nearly everyone improves within a month of onset of the pain.
Back pain is pain in any region of the back. It is divided into neck pain (cervical), middle back pain (thoracic), lower back pain (lumbar) or coccydynia (tailbone or sacral pain) based on the segment affected. The lumbar area is the most common area for pain, as it supports most of the weight in the upper body. Episodes of back pain may be acute, sub-acute, or chronic depending on the duration. The pain may be characterized as a dull ache, shooting or piercing pain, or a burning sensation. Discomfort can radiate into the arms and hands as well as the legs or feet, and may include numbness, or weakness in the legs and arms.
Low back pain can be caused by tumors, either benign or malignant, that originate in the bone of the spine or pelvis and spinal cord (primary tumors) and those which originate elsewhere and spread to these areas (metastatic tumors). Symptoms range from localized pain to radiating severe pain and loss of nerve and muscle function (even incontinence of urine and stool) depending on whether or not the tumors affect the nervous tissue. Tumors of these areas are detected using imaging tests, such as plain X-rays, nuclear bone scanning, and CAT and MRI scanning.

While the spine can maintain a natural curvature without lower back support provided by the seatback, the natural tendency for most people when sitting for a long period is to slouch forward. Twenty eight male participants (14 healthy individuals and 14 patients with LBP) between the ages of 21–50 were asked to participate in the study. Healthy individuals consisted of those who were free of LBP for the six months previous to the study, whereas patients with LBP had a history of LBP for at least three consecutive days over the last three consecutive weeks prior to testing. Individuals with a known neurological disorder, scoliosis or other deformity, inflammatory or degenerative arthropathy, connective tissue disease, or a history of spinal surgery were excluded from the study. Individuals with current or previous neck pain in the past three weeks were also excluded. Participants were asked to avoid engaging in any type of resistive exercise for the 48 hours prior to testing. All participants signed the informed consent form. The procedures used were in accordance with the institutional research ethics board. The clinical trial was registered at ClinicalTrials.gov (NCT00754585). Data were collected in the Biomechanics and Elastography laboratory at the Canadian Memorial Chiropractic College (CMCC).
Manual therapy. Typically this includes several sessions of massage, spinal mobilisation and/or spinal manipulation. With spinal mobilisation the therapist moves the joints of the spine around in their normal movement range. In spinal manipulation, the therapist moves joints beyond the usual range of movement.
There is moderate quality evidence that suggests the combination of education and exercise may reduce an individual’s risk of developing an episode of low back pain. Lesser quality evidence points to exercise alone as a possible deterrent to the risk of the onset of this condition.
