Bone scans are used to detect and monitor infection, fracture, or disorders in the bone. Scanner-generated images can be used to identify specific areas of irregular bone metabolism or abnormal blood flow, as well as to measure levels of joint disease.

Because back pain is so common, numerous products promise to prevent or relieve your back pain. But, there’s no definitive evidence that special shoes, shoe inserts, back supports, specially designed furniture or stress management programs can help. In addition, there doesn’t appear to be one type of mattress that’s best for people with back pain. It’s probably a matter of what feels most comfortable to you.

There are many different types of pain. Acute back pain is defined as severe but lasting a short period of time. Chronic back pain usually occurs every day. It can be severe, but may be characterized as mild, deep, achy, burning, or electric-like. Back pain that travels into another part of the body, such as the leg may be consider radicular pain, particularly when it radiates below the knee. This scenario is commonly called a lumbar radiculopathy. Fortunately, not all occurrences of back pain include leg pain!

In most cases of low back pain, medical consensus advises not seeking an exact diagnosis but instead beginning to treat the pain.[22] This assumes that there is no reason to expect that the person has an underlying problem.[22] In most cases, the pain goes away naturally after a few weeks.[22] Typically, people who do seek diagnosis through imaging are not likely to have a better outcome than those who wait for the condition to resolve.[22]

“The Pain Perplex,” a chapter in the book Complications, by Atul Gawande. Gawande’s entire book is worth reading, but his chapter on pain physiology is certainly the best summary of the subject I have ever read, and a terrific reminder that good writing for a general audience can be just as illuminating for professionals. Anyone struggling with a pain problem should buy the book for this chapter alone, though you are likely to enjoy the whole thing. Much of the chapter focuses on one of the most interesting stories of low back pain I’ve read, and it is a responsible and rational account — although Gawande, like most doctors, seems to be unaware of the clinical significance, or even existence, of myofascial trigger points.

Imaging is not typically needed in the initial diagnosis or treatment of back pain. However, if there are certain red flag symptoms present plain radiographs (x-ray), CT scan, or magnetic resonance imaging (MRI) may be recommended. These red flags include:[23][8]

Women may have acute low back pain from medical conditions affecting the female reproductive system, including endometriosis, ovarian cysts, ovarian cancer, or uterine fibroids.[28] Nearly half of all pregnant women report pain in the lower back or sacral area during pregnancy, due to changes in their posture and center of gravity causing muscle and ligament strain.[29]

Injections. If other measures don’t relieve your pain and if your pain radiates down your leg, your doctor may inject cortisone — an anti-inflammatory medication — or numbing medication into the space around your spinal cord (epidural space). A cortisone injection helps decrease inflammation around the nerve roots, but the pain relief usually lasts less than a few months.

2017 — Science update: Added a small but key new point and citation about genetic factors in degeneration; and an important new citation about the disconnect between imaging results and pain. [Section: Diagnosis: Your low back is not fragile!]
