I recently went on a coast to coast, 6,000 mile tour, and decided to try an experiment: I avoided sitting as much as I could. For example, in one room, I placed the mini fridge on the desk to make a modified standing desk. In the other room, I used a waste paper basket on another desk. By doing this, I was able to reduce my 12 to 14 hours of daily sitting to just under an hour.
Back pain can be divided into non-radicular pain and radiculopathy. Radiculopathy occurs when there is irritation in the nerve root, causing neurologic symptoms, such as numbness and tingling. Disk herniation and foraminal stenosis are the most common causes of radiculopathy. Non-radicular back pain is most commonly caused by injury to the spinal muscles or ligaments, degenerative spinal disease, or a herniated disk. Spondylosis, or spinal degeneration, occurs when the intervertebral disc undergoes degenerative changes, causing the disc to fail at cushioning the vertebrae. The space between the vertebrae becomes more narrow, resulting in compression and irritation of the nerves.

Plain X-rays are generally not considered useful in the evaluation of acute back pain, particularly in the first 30 days. In the absence of red flags, their use is discouraged. Their use is indicated if there is significant trauma, mild trauma in those older than 50 years of age, people with osteoporosis, and those with prolonged steroid use. Do not expect an X-ray to be taken.
Fibromyalgia – In addition to back pain, there are usually other areas of pain and stiffness in the trunk, neck, shoulders, knees and elbows. Pain may be either a general soreness or a gnawing ache, and stiffness is often worst in the morning. People usually complain of feeling abnormally tired, especially of waking up tired, and they have specific areas that are painful to touch, called tender points.
Tests such as X-rays, scans or blood tests may be advised in certain situations. This is mainly if there are symptoms, or signs during a doctor’s examination, to suggest that there may be a serious underlying cause for the back pain.
If back pain is severe, your doctor may try other medications that focus on different parts of the pain response, such as gabapentin or amitriptyline, a tricyclic antidepressant. The latter may work better for nerve-related pain.

If you see a doctor for back pain, he (or she) may use terms such as thoracic, lumbar, lumbosacral, or sacrum. The point is, back pain is a large topic covering many different regions (or levels) of the spine.

My original inspiration for this tutorial was Dr. John Sarno’s 1984 book Mind over back pain. (His more recent Healing back pain makes too many empty promises. See my review.) However, as much as I respect Dr. Sarno’s early work, there are at least three reasons why this tutorial is better than his books: (1) I make a much more airtight case against the conventional medical myths of back pain than Dr. Sarno does; (2) I also build a much better case for the real causes of back pain, heavily referencing more credible sources than Dr. Sarno does; (3) and I offer many more practical suggestions than Dr. Sarno does, instead of focusing exclusively on the psychological factors. Doctors can use several tests to look inside you to get an idea of what might be causing the back pain. No single test is perfect in that it identifies the absence or presence of disease 100% of the time.

Acute pain. This type of pain typically comes on suddenly and lasts for a few days or weeks, and is considered a normal response of the body to injury or tissue damage. The pain gradually subsides as the body heals.
The management of low back pain often includes medications for the duration that they are beneficial. With the first episode of low back pain the hope is a complete cure; however, if the problem becomes chronic, the goals may change to pain management and the recovery of as much function as possible. As pain medications are only somewhat effective, expectations regarding their benefit may differ from reality, and this can lead to decreased satisfaction.
Computerized tomography (CT) is used to see spinal structures that cannot be seen on conventional x-rays, such as disc rupture, spinal stenosis, or tumors. Using a computer, the CT scan creates a three-dimensional image from a series of two dimensional pictures.
Sitting all day is hazardous, too. It puts more pressure on disks and vertebrae than standing or walking, Dr. Katz says. Alleviate the tension with an office makeover. Herbal medicines, as a whole, are poorly supported by evidence. The herbal treatments Devil’s claw and white willow may reduce the number of individuals reporting high levels of pain; however, for those taking pain relievers, this difference is not significant. Capsicum, in the form of either a gel or a plaster cast, has been found to reduce pain and increase function.
The lumbar region is simply the lower section of your back, also called the lower spine. It has a natural curve that bends slightly inward. This curvature, coupled with sedentary lifestyles and poor posture, mean many people slouch in their seats—forcing the lumbar outward and causing lower back pain. Over time, this pain can become chronic.
Acute back pain comes on suddenly but improves over time; chronic pain worsens and can last months. If you don’t feel better after three to four days, see your primary care physician. She may refer you to a neurologist or a neurosurgeon if the pain is nerve related; an orthopedist, osteopath, or chiropractor for musculoskeletal problems; a rheumatologist for joint problems; or a physical therapist or physiatrist for rehabilitation exercises.
