Sure, sky-high stilettos are a no-no, but it turns out that flats can cause trouble, too. Sandals and flip-flops often provide little, if any, arch support. Continuous wear can lead to back, knee, and foot problems down the line, says Megan Tabor, a chiropractor at the Osher Clinical Center at Brigham and Women’s Hospital. But don’t worry: You needn’t settle for all function and no flair. Alternate styles throughout the week — from high to low, sneakers to sandals — and avoid wearing a particular pair every day. Shoes should fit properly and offer good arch and heel support, Tabor says. If you walk to work or the bus stop, wear shock-absorbing sneakers, then slip on cuter kicks once you get to the office. Your purse could also be to blame, especially if it’s huge and you’re lugging it on one shoulder. Try a tote with a wide, padded strap; carry it messenger style; and lighten the load. The mean (SD) age for the healthy and LBP groups were 26.3 ± 2.1 years and 27.8 ± 6.1 years, respectively. The mean (SD) height and weight for the healthy and LBP groups were 174.6 ± 13.5 cm and 176.0 ± 9.7 cm (height) and 81.8 ± 11.8 kg and 80.7 ± 12.3 kg (weight), respectively. Furthermore, the mean (SD) of the intensity of the LBP in the patient group were 3.4 (1.6) out of 10 on the VAS. The posture data for only 25 participants were used (11 healthy individuals and 14 patients with LBP) as it was determined during data analysis that the markers had moved during collection for three participants and the data were not accurate. The comfort data from all 28 participants were used for analysis.

I recently started working from home more often and realized that my home desk chair isn’t the most supportive. My back was starting to bother me. I purchased this support and added it to my chair. Now my posture is much better and my back doesn’t hurt anymore. It’s just a simple mesh device but it stays put and doesn’t move around on my chair. A complete medical history and physical exam can usually identify any serious conditions that may be causing the pain. During the exam, a health care provider will ask about the onset, site, and severity of the pain; duration of symptoms and any limitations in movement; and history of previous episodes or any health conditions that might be related to the pain. Along with a thorough back examination, neurologic tests are conducted to determine the cause of pain and appropriate treatment. The cause of chronic lower back pain is often difficult to determine even after a thorough examination.

Massage therapy does not appear to provide much benefit for acute low back pain. A 2015 Cochrane review found that for acute low back pain massage therapy was better than no treatment for pain only in the short-term. There was no effect for improving function. For chronic low back pain massage therapy was no better than no treatment for both pain and function, though only in the short-term. The overall quality of the evidence was low and the authors conclude that massage therapy is generally not an effective treatment for low back pain.
Example: a friend of mine went to the hospital after a motorcycle accident. He’d flown over a car and landed hard on his head. Bizarrely, he was sent home with very little care, and no imaging of his back, even though he was complaining of severe lower back pain. A doctor reassured him that it was just muscle spasms. (This all happened at a hospital that was notorious for being over-crowded and poorly run.) The next day, still in agony, he went to see a doctor at a walk-in clinic, who immediately took him for an x-ray… which identified a serious lumbar fracture and imminent danger of paralysis. He had been lucky to get through the night without disaster! He was placed on a spine board immediately and sent for surgery. The moral of the story? Sometimes, when you’ve had a major trauma and your back really hurts, it’s because your back is broken. BACK TO TEXT
The presence of any acute nerve dysfunction should also prompt an immediate visit. These would include the inability to walk or inability to raise or lower your foot at the ankle. Also included would be the inability to raise the big toe upward or walk on your heels or stand on your toes. These might indicate an acute nerve injury or compression. Doctors lack the skills and knowledge needed to care for most common aches, pains, and injury problems, especially the chronic cases, and even the best are poor substitutes for physical therapists. Low back pain may be classified based on the signs and symptoms. Diffuse pain that does not change in response to particular movements, and is localized to the lower back without radiating beyond the buttocks, is classified as nonspecific, the most common classification. Pain that radiates down the leg below the knee, is located on one side (in the case of disc herniation), or is on both sides (in spinal stenosis), and changes severity in response to certain positions or maneuvers is radicular, making up 7% of cases. Pain that is accompanied by red flags such as trauma, fever, a history of cancer or significant muscle weakness may indicate a more serious underlying problem and is classified as needing urgent or specialized attention.

Twenty eight male participants (14 healthy individuals and 14 patients with LBP) between the ages of 21–50 were asked to participate in the study. Healthy individuals consisted of those who were free of LBP for the six months previous to the study, whereas patients with LBP had a history of LBP for at least three consecutive days over the last three consecutive weeks prior to testing. Individuals with a known neurological disorder, scoliosis or other deformity, inflammatory or degenerative arthropathy, connective tissue disease, or a history of spinal surgery were excluded from the study. Individuals with current or previous neck pain in the past three weeks were also excluded. Participants were asked to avoid engaging in any type of resistive exercise for the 48 hours prior to testing. All participants signed the informed consent form. The procedures used were in accordance with the institutional research ethics board. The clinical trial was registered at ClinicalTrials.gov (NCT00754585). Data were collected in the Biomechanics and Elastography laboratory at the Canadian Memorial Chiropractic College (CMCC).
