Opioid analgesics: These drugs are considered an option for pain control in acute back pain. The use of these medications is associated with serious side effects, including dependence, sedation, decreased reaction time, nausea, and clouded judgment. One of the most troublesome side effects is constipation. This occurs in a large percentage of people taking this type of medication for more than a few days. A few studies support their short-term use for temporary pain relief. Their use, however, does not speed recovery.
A good lumbar support is quite simple. It should be comfortable, and beneficial to the health of your back. However, as simple as these requirements are, the back itself is a complicated place. There are a wide variety of reasons for people to seek lumbar support, and these reasons stem from complications deriving from lumbar related back problems.

Nonsteroidal anti-inflammatory drugs (NSAIDS) relieve pain and inflammation and include OTC formulations (ibuprofen, ketoprofen, and naproxen sodium). Several others, including a type of NSAID called COX-2 inhibitors, are available only by prescription. Long-term use of NSAIDs has been associated with stomach irritation, ulcers, heartburn, diarrhea, fluid retention, and in rare cases, kidney dysfunction and cardiovascular disease. The longer a person uses NSAIDs the more likely they are to develop side effects. Many other drugs cannot be taken at the same time a person is treated with NSAIDs because they alter the way the body processes or eliminates other medications.
Approximately 98 percent of people with back pain are diagnosed with nonspecific acute back pain in which there is no serious underlying pathology. Nearly 2 percent are comprised by metastatic cancers, while serious infections such as spinal osteomyelitis and epidural abscesses account for fewer than 1 percent.

Most acute low back pain fades steadily — up to 90% of it, for uncomplicated cases.13 So does a lot of so-called “chronic” low back pain!14 But when you don’t recover, many of the therapeutic options — things like surgery for a herniated disc — cause anxiety that is unnecessary and harmful. Prescription opioid medications, such as oxycodone (OxyContin), may relieve pain, but doctors are typically reluctant to prescribe them because people rapidly develop a tolerance and the drugs can be diverted for illicit use in the community, Maher said.

It’s much the same story with back pain: We don’t really know what to do about it. There aren’t good treatments for it, Vos said, and some of the things that people use to try to manage it—like prescription opioids—have only morphed into health crises in their own right.

Paget’s disease of the bone is a condition of unknown cause in which the bone formation is out of synchrony with normal bone remodeling. This condition results in abnormally weakened bone and deformity and can cause localized bone pain, though it often causes no symptoms. Paget’s disease is more common in people over the age of 50. Heredity (genetic background) and certain unusual virus infections have been suggested as causes. Thickening of involved bony areas of the lumbar spine can cause the radiating lower extremity pain of sciatica.

Low back pain results in large economic costs. In the United States, it is the most common type of pain in adults, responsible for a large number of missed work days, and is the most common musculoskeletal complaint seen in the emergency department. In 1998, it was estimated to be responsible for $90 billion in annual health care costs, with 5% of individuals incurring most (75%) of the costs. Between 1990 and 2001 there was a more than twofold increase in spinal fusion surgeries in the US, despite the fact that there were no changes to the indications for surgery or new evidence of greater usefulness. Further costs occur in the form of lost income and productivity, with low back pain responsible for 40% of all missed work days in the United States. Low back causes disability in a larger percentage of the workforce in Canada, Great Britain, the Netherlands and Sweden than in the US or Germany.
“For many years, I’ve had a very severe backache, and I don’t really seek care for it because it doesn’t really help,” he said. So it’s likely back pain will be with Vos—and many others around the world— “for a long time to come.”
About 80 percent of people in the United States experience at least one bout of lower-back pain in their lives, according to the National Institute of Neurological Disorders and Stroke. The back can get out of whack very easily because it supports most of the body’s weight. People ages 30 to 50 tend to be more likely to suffer from back pain, possibly because they spend large amounts of their day sitting, with the occasional too-vigorous workout that can cause injury.
Back pain can usually be managed successfully with the simple treatments listed above, but about 10 percent of people have ongoing problems. Back surgery is usually a last resort in people who have an underlying cause for their pain.
