The Sony World Photography Awards, an annual competition hosted by the World Photography Organisation, just announced its shortlist of winners for 2018. This year’s contest attracted nearly 320,000 entries from more than 200 countries. The organizers have again been kind enough to share some of their shortlisted and commended images with us, gathered below. Overall winners are scheduled to be announced on April 19. All captions below come from the photographers.

The big idea of classification-based cognitive functional therapy (CB-CFT or just CFT) is that most back pain has nothing to do with scary spinal problems and so the cycle of pain and disability can be broken by easing patient fears and anxieties. For this study, CFT was tried with 62 patients and compared to 59 who were treated with manual therapy and exercise. The CFT group did better: a 13-point boost on a 100-point disability scale, and 3 points on a 10-point pain scale. As the authors put it for BodyInMind.org, “Disabling back pain can change for the better with a different narrative and coping strategies.” These results aren’t proof that the confidence cure works, but they are promising.

As the pain subsides the physical therapist may introduce some flexibility and strength exercises for the back and abdominal muscles. Techniques on improving posture may also help. The patient will be encouraged to practice the techniques regularly, even after the pain has gone, to prevent back pain recurrence.

More than 90% of people with back pain get better after conservative treatment. Only 5% of people with back pain will have symptoms for more than 12 weeks and for most of these people, the cause is not serious.

Bed rest pillows are a common accessory in many bedrooms for anyone who reads or works on a laptop in bed. They help promote a healthier posture so you don’t have to slump over while reading or working. These pillows may look familiar from your childhood – almost everyone has probably had a backrest pillow at some point growing up. They are pillows with backs and armrests attached – backrest pillows have come a long way in both design and style.

On any given day, almost 2% of the entire US work force is disabled by back pain. In people under 40 years of age, back pain is the most common reason for the inability to perform daily tasks. It also is the direct cause of enormous healthcare expenses, with estimates as high as $60 billion annually.

CBT (cognitive behavioral therapy) – CBT can help patients manage chronic back pain. The therapy is based on the principle that the way a person feels is, in part, dependent on the way they think about things.

“Lower back pain” refers to any pain or discomfort experienced in the bottom region of the spine, which is known as the lumbar spine. This region supports most of the upper body’s weight. Additionally, this specific type of back pain is extremely widespread, affecting around 80 percent of adults and ranking as the number one cause of job-related disability. Luckily, there are many effective treatments to alleviate or altogether eliminate lower back pain.

At the beginning of the data collection, participants were asked to stand in a neutral position “with their arms by their side, weight evenly balanced, and looking straight ahead”. Kinematic data were collected over 30 seconds of neutral static stance for comparison against the seated conditions. Participants sat in the office chair and in the same seat pan but with a lumbar support pillow (“Logic Back™”, Mediflow Inc., Toronto) to test the effect of the back rest profile on comfort and lumbar and thoracolumbar postures. The lumbar support is a portable device, convex in the anterior direction and contoured with an arched opening above the seat pan, that provides space for the bulk of the posterior pelvic tissues (Figure 1). The back frame of the device is constructed of a solid plastic, curved side-to-side and is relatively rigid. The frame acts as a bow which is strung anteriorly by four adjustable straps. These straps provide an elastic, anterior projection above the buttock tissues. A band affixes the device to the chair back. The lumbar support was “fitted” to each participant prior to testing by having the individual sit in the chair in a relaxed fashion, with the hips and knees flexed to 90°, feet flat and looking straight ahead. The pelvis was pushed all the way back into the aperture of the pillow, and the individual’s lumbar spine rested against the back rest. The straps were tensioned to participant preference.

Skeletal muscle relaxers may also be used.[26] Their short term use has been shown to be effective in the relief of acute back pain.[36] However, the evidence of this effect has been disputed, and these medications do have negative side-effects.[34]

Bone and joint conditions: Bone and joint conditions that lead to low back pain include those existing from birth (congenital), those that result from wear and tear (degenerative) or injury, and those that are due to inflammation of the joints (arthritis).

Whether you choose a full-seat back cushion or a smaller back support, always ensure the material is flush against the small of your back. The support shouldn’t be pressing into your sides or pinching any nerves, as this can cause more damage over time.

The amplitude of the postural difference in the lumbar region was small, in the order of 2-3°, and it is unknown whether this degree of change is associated with clinical benefit. Although not directly applicable to the current study, small changes in the order of 2-3° may considerably influence the compressive load at L4-L5 [9], particularly when taking into account the cumulative effect of spinal loading throughout an entire work day. The effect of angular change in the low back, assuming erect seated posture, is multiplied by its action on the position of the center of upper body mass by means of a relatively long moment arm approximating 20% of body height [9]. While it has been shown that the longissimus/iliocostalis muscle fibre angles change when the lumbar spine flexes fully forward [29], the change in orientation with a small amount of flexion is unknown. A cadaveric study has shown that minimal amount of flexion removes the stress peaks in the posterior annulus but may increase stresses in the nucleus and anterior annulus [30].

Low back pain has been with humans since at least the Bronze Age. The oldest known surgical treatise – the Edwin Smith Papyrus, dating to about 1500 BCE – describes a diagnostic test and treatment for a vertebral sprain. Hippocrates (c. 460 BCE – c. 370 BCE) was the first to use a term for sciatic pain and low back pain; Galen (active mid to late second century CE) described the concept in some detail. Physicians through the end of the first millennium did not attempt back surgery and recommended watchful waiting. Through the Medieval period, folk medicine practitioners provided treatments for back pain based on the belief that it was caused by spirits.[96]

Nonsteroidal anti-inflammatory medications (NSAIDs) are the mainstay of medical treatment for the relief of back pain. Ibuprofen, naproxen, ketoprofen, and many others are available. No particular NSAID has been shown to be more effective for the control of pain than another. However, your doctor may switch you from one NSAID to another to find one that works best for you.

People routinely have no pain despite the presence of obvious arthritic degeneration, herniated discs, and other seemingly serious structural problems like stenosis and spondylolistheses. This surprising contradiction has been made clear by a wide variety of research over the years, but the most notable in recent history is Brinjikji 2015. There are painful spinal problems, of — which was also shown by Brinjikji et al in a companion paper — but they are mostly more rare and unpredictable than most people suspect, and there are many fascinating examples of people who “should” be in pain but are not, and vice versa. Opioid analgesics: These drugs are considered an option for pain control in acute back pain. The use of these medications is associated with serious side effects, including dependence, sedation, decreased reaction time, nausea, and clouded judgment. One of the most troublesome side effects is constipation. This occurs in a large percentage of people taking this type of medication for more than a few days. A few studies support their short-term use for temporary pain relief. Their use, however, does not speed recovery.

  1. Fitness level: Back pain is more common among people who are not physically fit. Weak back and abdominal muscles may not properly support the spine. “Weekend warriors”—people who go out and exercise a lot after being inactive all week—are more likely to suffer painful back injuries than people who make moderate physical activity a daily habit. Studies show that low-impact aerobic exercise is beneficial for the maintaining the integrity of intervertebral discs.
    In human anatomy the five vertebrae in the lumbar region of the back are the largest and strongest in the movable part of the spinal column, and can be distinguished by the absence of a foramen in the transverse process, and by the absence of facets on the sides of the body. In most mammals, the lumbar region of the spine curves outward.
    Well, at least there’s that! But most of what CR published was horrifyingly naive and misleading. I scanned this issue in a grocery store lineup and was rolling my eyes within seconds. And then fuming: it seems like the flood of misinformation about back pain is infinite! I’ve been actively debunking back pain myths for about 15 years now, and the need for it has barely changed in all that time. So-called information like this, reaching a massive audience, seriously exacerbates the problem.
