There are many different conditions that cause or contribute to low and lower back pain. Many involve nerve compression (eg, pinched nerve) that may generate pain and other symptoms. Types of spinal disorders include trauma-related and degenerative; meaning age-related. Some of these spinal problems are listed below.

With sciatica, you will feel numbness and pain down the back of the leg, side of the calf, and possibly into the side of the foot, but not necessarily much back pain. The exact area where you will feel numbness depends on the nerve root that is affected; the numbness could be in the inner ankle, big toe, heel, outer ankle, outer leg, or a combination of them. When the nerve roots’ motor function is damaged by disc herniation, you may also experience weakness in certain parts of the leg and foot.

Muscle relaxants: Muscle spasm is not universally accepted as a cause of back pain, and most relaxants have no effect on muscle spasm. No additional benefit is gained by using muscle relaxants in combination with NSAIDs over using NSAIDs alone. Muscle relaxants cause drowsiness in up to 30% of people taking them. Their use is not routinely recommended.

During the exam, a health care provider will ask about the onset, site, and severity of the pain; duration of symptoms and any limitations in movement; and history of previous episodes or any health conditions that might be related to the pain. Along with a thorough back examination, neurologic tests are conducted to determine the cause of pain and appropriate treatment. The cause of chronic lower back pain is often difficult to determine even after a thorough examination.

There are several types available, including operant conditioning, which uses reinforcement to reduce undesirable behaviors and increase desirable behaviors; cognitive behavioral therapy, which helps people identify and correct negative thinking and behavior; and respondent conditioning, which can modify an individual’s physiological response to pain. Medical providers may develop an integrated program of behavioral therapies.
