The bony lumbar spine is designed so that vertebrae “stacked” together can provide a movable support structure while also protecting the spinal cord from injury. The spinal cord is composed of nervous tissue that extends down the spinal column from the brain. Each vertebra has a spinous process, a bony prominence behind the spinal cord, which shields the cord’s nervous tissue from impact trauma. Vertebrae also have a strong bony “body” (vertebral body) in front of the spinal cord to provide a platform suitable for weight bearing of all tissues above the buttocks. The lumbar vertebrae stack immediately atop the sacrum bone that is situated in between the buttocks. On each side, the sacrum meets the iliac bone of the pelvis to form the sacroiliac joints of the buttocks.

However, once the pain has eased or gone it is common to have further bouts (recurrences) of pain from time to time in the future. Also, it is common to have minor pains on and off for quite some time after an initial bad bout of pain. In a small number of cases the pain persists for several months or longer. This is called chronic back pain (discussed in more detail later).

Application of heat or cold may help (see Treatment of Pain and Inflammation). Cold is usually preferred to heat during the first 2 days after an injury. Ice and cold packs should not be applied directly to the skin. An ice pack should be enclosed (for example, in plastic) and placed over a towel or cloth. The ice is removed after 20 minutes, then reapplied for 20 minutes over a period of 60 to 90 minutes. This process can be repeated several times during the first 24 hours. Heat, using a heating pad, can be applied for the same periods of time. Because the skin on the back may be insensitive to heat, heating pads must be used cautiously to prevent burns. People should not use a heating pad at bedtime to avoid the risk of falling asleep with the pad still on their back.

An estimated 75 to 85 percent of all Americans will experience some form of back pain during their lifetime. Although low back pain can be quite debilitating and painful, in about 90 percent of all cases, pain improves without surgery. However, 50 percent of all patients who suffer from an episode of low back pain will have a recurrent episode within one year.
Endoscopy The medical history should focus on both triggering and alleviating factors, as well as on the character of the pain. Signs and symptoms such as increased pain with Valsalva maneuvers, straight-leg-raising symptoms, the tendency for the pain to radiate into the buttock or leg, the presence of weakness or sensory deficit, and bowel or bladder urgency or incontinence are associated with neurologic causes of low back pain.
A ruptured or herniated disk can cause low back pain. A disk has a tough covering and a soft, jelly-like interior. If a disk is suddenly squeezed by the vertebrae above and below it (as when lifting a heavy object), the covering may tear (rupture), causing pain. The interior of the disk can squeeze through the tear in the covering, so that part of the interior bulges out (herniates). This bulge can compress, irritate, and even damage the spinal nerve root next to it, causing more pain. A ruptured or herniated disk that affects nerves commonly causes sciatica. Imaging studies such as MRI may show that disks bulge, including in people who have no symptoms; this bulging usually causes no symptoms or problems.
